These findings suggest that free antibiotic concentration at target sites is a more appropriate pharmacokinetic input in a PK/PD model for predicting therapeutic efficacy of an antibiotic. Pharmacodynamics. The bacterial time-kill curves of cefpodoxime and cefixime against four bacterial strains (Haemophilus influenzae, Moraxella catarrhalis, Streptococcus pneumoniae-penicillin sensitive, and Streptococcus pneumoniae penicillin-intermediate) were compared in an in vitro infection model in which the human pharmacokinetic profile of unbound antibiotic was simulated. Both antibiotics have similar high potency against H. influenzae and M. catarrhalis, while cefpodoxime has about 10-fold higher potency against S. pneumonia strains than cefixime. This approach offers more detailed information than minimum inhibitory concentration (MIC) does about the time course of antibacterial activity of an antibiotic. The developed PK/PD model (sigmoidal E[subscript max] model) effectively describes the bacterial time-kill curves of antibiotics against four bacterial strains. Moreover, simulations based on this model show potential for predicting the antibacterial activity of the antibiotic in different clinical settings.
